Women's Self Defense Reservation Process

Females, Ages 16 and up are welcome to our free, Women's Self Defense classes structured specifically for Ladies

Classes are offerred Tuesday evenings from 6 - 7 pm.

We limit reservations to 14 per class to ensure the highest integrity of our program.

After completing the accompanying form, please complete the subsequnt acceptance of responsibility, and then select which Tuesday you'd like to attend on the following page. Thank you

I understand and acknowledge that an inherent risk of physical injury exists with participation in any sport or martial art, combat or otherwise. By training at The Jiu Jitsu Mill I assert that I am in good physical health and I voluntarily assume all risks, physical or otherwise related to training at or participating in activities at The Jiu Jitsu Mill.
COVID Screening
I hereby certify that I have either 1) had COVID-19 within the past three months. I am symptom free and it has been in excess of 10 days since the onset of my symptoms. 2) Been vaccinated for COVID-19. 3) Not had COVID-19 or been vaccinated but have not experienced any of the following within the past 10 days: fever, shortness of breath, cough, flu like symptoms such as headache, fatigue or gastrointestinal or loss of taste or smell. I hereby certify that I have not within the past 14 days had contact with anyone confirmed to have COVID-19.

Personal Belongings Acknowledgement
I agree that The Jiu Jitsu Mill is in no way responsible for the safekeeping of my personal belongings. I understand that any soiled training gear or clothing left at the gym will be discarded immediately and that any other belongings will be kept for one week prior to discarding.
