Remember you should be wearing your gi and belt only for class. Respect your uniform and keep it clean.

1. TWENTY-FOUR HOUR NOTICE IS REQUIRED TO RESCHEDULE ANY REGULARLY SCHEDULED PRIVATE LESSON.
2. FOR PRIVATE LESSONS, YOU ONLY USE CREDITS FOR THE LESSONS YOU TAKE. ONE CREDIT = 20 MINUTE LESSON.
3. I AGREE TO PARTNER WITH THE OMMA INSTRUCTORS TO ENSURE STUDENT(S) ABIDE BY ALL CLASS RULES.
4. I AGREE TO TIMELY PAYMENT OF MY CLASS FEES MONTHLY (OR AS ARRANGED) ACCORDING TO THE PAYMENT POLICY.
5. PLEASE MAKE SURE THE INSTRUCTOR KNOWS AHEAD OF TIME IF YOU WILL NOT BE AVAILABLE FOR YOUR REGULAR PRIVATE LESSON TIME. IT MIGHT BE POSSIBLE TO RESCHEDULE BUT THERE ARE NO GUARANTEES FOR A DIFFERENT TIME/DAY THAT WEEK.
6. I AGREE TO PARTNER WITH THE OMMA INSTRUCTORS TO ENSURE STUDENT(S) WILL TREAT THE MARTIAL ARTS LEARNED WITH HONOR AND RESPECT AND WILL NOT USE THEM OUTSIDE THE CLASSROOM OR THEIR OWN PRIVATE PRACTICE AREA WITHOUT BEING UNDER THE DIRECT SUPERVISION OF THE INSTRUCTOR.
7. I UNDERSTAND THAT AS PART OF THIS CHRISTIAN MARTIAL ARTS PROGRAM, OMMA STUDENTS PARTICIPATE IN SUCH SPIRITUAL DISCIPLINES AS PRAYER, WORSHIP, BIBLE READING, BIBLE STUDY, SCRIPTURE MEMORIZATION AND ACTS OF SERVICE.
8. I AUTHORIZE OMMA TO USE MY CHILD’S OR MY IMAGE IN PHOTOGRAPHY FOR OMMA NEWSLETTERS, BROCHURES, ADVERTISEMENTS, DVD/VIDEO AND ON THE OMMA WEBSITE. I UNDERSTAND THAT WE WILL NOT BE COMPENSATED IN ANY WAY FOR THE USE OF THESE PHOTOGRAPHS. NAMES WILL NOT BE USED WITHOUT ADDITIONAL CONSENT.
9. I AGREE THAT OMMA IS NOT RESPONSIBLE FOR ANY INJURY TO SAID STUDENT WHILE TAKING ON MISSION MARTIAL ARTS CLASSES, PRIVATE LESSONS, OR TESTS.
