I/we hereby assume all responsibility to participant in classes. I/we waive any and all claims of injury, damages, or loss to myself or personal property and hold no one responsible for any accident or injury associated with this physical activity. I/we agree to hold harmless said institution, its principals and agents, representatives, employees, and students from any and all claims.
Furthermore, I/we know of no impairment of deficiency in physical health or otherwise that would limit or prohibit participant from participation in practice sessions, lessons, and/or game competition. I/we agree to make known to the instructor(s) and studio officials any change in physical health or any other consideration that would limit participation.
I hereby certify and represent that I am in sound mental and physical condition. I agree that all mental and physical recommendations given to me, are undertaken by me, at the sole risk of myself and that Salem-Keizer Brazilian Jiu-Jitsu shall not be liable for any claims for injuries or damages whatsoever to my person, my child or property arising out of, or in connection with, the training and recommendations given by Salem-Keizer Brazilian Jiu-Jitsu, its employees, and subcontractors from all claims by, or liability to me. I agree to a photo and video release for all photos and videos taken in Salem-Keizer Brazilian Jiu-Jitsu Academy to be posted on Facebook and other social media and advertising platforms. I also certify and represent that as a student of Salem-Keizer Brazilian Jiu-Jitsu, I agree to abide by all rules and regulations of Salem-Keizer Brazilian Jiu-Jitsu, and that failure to do so may result in my child’s expulsion from training and forfeiture of all moneys paid to Salem-Keizer Brazilian Jiu-Jitsu.I agree to abide by the rules and regulations governing the conduct and operation of instruction sessions, students, observers, in order to promote efficiency, safety, and recognition of instruction certification, as prescribed by the Gym.
The novel coronavirus (“COVID-19”), has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. While rules, guidance, and personal discipline may reduce this risk, the risk of serious illness and death does exist. Salem-Keizer Brazilian Jiu-Jitsu cannot completely mitigate the transfer of communicable diseases like COVID-19 especially when involved in Jiu-Jitsu, which is an indoor full contact sport. I understand that my child participating in indoor full contact Jiu-Jitsu increases the likelihood of transmission of COVID-19. Participation in Jiu-Jitsu includes possible exposure to and illness, injury, or death from infectious diseases including COVID-19. I agree to quarantine if exposed to COVID-19 or isolate if my child contracts COVID-19.
In consideration for providing my child the opportunity to participate in Jiu-Jitsu and any related transportation to and from Jiu-Jitsu events, both my child and I voluntarily agree to waive and discharge any and all claims against Salem-Keizer Brazilian Jiu-Jitsu and release it from liability for any exposure to or illness or injury from an infectious disease including COVID-19, including claims for any negligent actions of Salem-Keizer Brazilian Jiu-Jitsu or its employees or agents, to the fullest extent allowed by law, for myself, my child, our estates, our heirs, our administrators, our executors, our assignees, and our successors.
I also agree to release, exonerate, discharge and hold harmless Salem-Keizer Brazilian Jiu-Jitsu, the individual members thereof, and all officers, agents, employees, volunteers, and representatives from all liability, claims, causes of action, or demands, including attorney fees, fines, fees, or other costs (e.g. medical costs) arising out of any exposure to or illness or injury from an infectious disease including COVID-19, which may result from or in connection with my child’s participation in Jiu-Jitsu.
I further certify and represent that I have the legal authority to waive, discharge, release, and hold harmless the released parties on behalf of myself and the above-named student.
I certify that I have read this document in its entirety and fully understand its contents. In exchange for the opportunity to participate in Jiu-Jitsu, the above-named student and I freely and voluntarily assume all risks of such hazards and notwithstanding such, release Salem-Keizer Brazilian Jiu-Jitsu from all liability for any loss regardless of cause, and claims arising from the student's participation in Jiu-Jitsu.

Students under the age of 18 must must be accompanied by a parent or legal guardian on their first visit.
