Remember you should be wearing your gi and belt only for class. Respect your uniform and keep it clean.
1. TWENTY-FOUR HOUR NOTICE IS REQUIRED TO RESCHEDULE ANY REGULARLY SCHEDULED PRIVATE LESSON.
2. A PAUSE CAN BE PLACED ON YOUR TUITION WHILE ON VACATION IF YOU ARRANGE TO PUT A VACATION HOLD ON YOUR ACCOUNT AT LEAST TWO WEEK AHEAD OF TIME.
3. I AGREE TO PARTNER WITH THE OMMA INSTRUCTORS TO ENSURE STUDENT(S) ABIDE BY ALL CLASS RULES.
4. I AGREE TO TIMELY PAYMENT OF MY CLASS FEES MONTHLY (OR AS ARRANGED) ACCORDING TO THE PAYMENT POLICY.
5. ONE GROUP CLASS CAN BE MADE UP EITHER THE WEEK PRIOR TO AN ABSENCE OR THE WEEK FOLLOWING THE ABSENCE.
6. I AGREE TO PARTNER WITH THE OMMA INSTRUCTORS TO ENSURE STUDENT(S) WILL TREAT THE MARTIAL ARTS LEARNED WITH HONOR AND RESPECT AND WILL NOT USE THEM OUTSIDE THE CLASSROOM OR THEIR OWN PRIVATE PRACTICE AREA WITHOUT BEING UNDER THE DIRECT SUPERVISION OF THE INSTRUCTOR.
7. I UNDERSTAND THAT AS PART OF THIS CHRISTIAN MARTIAL ARTS PROGRAM, OMMA STUDENTS PARTICIPATE IN SUCH SPIRITUAL DISCIPLINES AS PRAYER, WORSHIP, BIBLE READING, BIBLE STUDY, SCRIPTURE MEMORIZATION AND ACTS OF SERVICE.
8. I AUTHORIZE OMMA TO USE MY CHILD’S OR MY IMAGE IN PHOTOGRAPHY FOR OMMA NEWSLETTERS, BROCHURES, ADVERTISEMENTS, DVD/VIDEO AND ON THE OMMA WEBSITE. I UNDERSTAND THAT WE WILL NOT BE COMPENSATED IN ANY WAY FOR THE USE OF THESE PHOTOGRAPHS. NAMES WILL NOT BE USED WITHOUT ADDITIONAL CONSENT.
9. I AGREE THAT OMMA IS NOT RESPONSIBLE FOR ANY INJURY TO SAID STUDENT WHILE TAKING ON MISSION MARTIAL ARTS VIRTUAL CLASSES, PRIVATE LESSONS, OR TESTS.
Due to the 2019-2020 outbreak of the novel Coronavirus (COVID-19), On Mission Martial Arts is taking extra precautions with the care of every person associated with On Mission Martial Arts. Due to this event, I hereby agree to the following: I understand COVID-19 symptoms (fever, fatique, dry cough, difficulty breathing) and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above within the last 14 days. I affirm that I, as well as all household members, have not been diagnosed with COVID-19 within the past 30 days. I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days. I affirm that I, as well as all household members, have not traveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections within the past 30 days. I understand that On Mission Martial Arts cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each person. I also agree that for future visits I will adhere to these same criteria in regards to COVID-19 symptoms and parameters mentioned above.
I agree that by accepting this digital waiver it will apply to all children and adults living in this household who are training with On Mission Martial Arts.
